Accidents, incidents and deviations that occur onboard are reported to the maritime incident reporting system, Insjö/Foresea. The reports are written onboard and sent to the Insjö system through the shipping company offices. You can find statistics on the system’s homepage about different kinds of incidents based on the type of ship and its operations, and obtain information about the underlying causes and consequences of an incident. Insjö is operated by the shipowner’s association in collaboration with the Swedish Transport Agency. Reporting forms and databases can be found at www.ipso.cc.
