Sometimes apparently small mistakes can have very large consequences. Three examples of such events are given below, in which what appeared to be an inconsequential mistake led to an accident or consequences that were important for operations. The three events have one thing in common: someone had forgotten to reset a switch or valve after work had been carried out on a system.
