The industry’s own accident reporting system, Insjö/Foresea, has received eleven reports about hot work, welding work, that caused or risked causing a fire. In ten cases there was either fire or smoke. On one occasion, welding work was being carried out on a loading device. Red-hot steel drops fell down on the tank top. An hour later, it was discovered that leaked hydraulic oil had been ignited and a large amount of smoke was produced. Only after extinguishing using powder and water was the situation finally under control.
In view of the above, the investigation unit wishes to make the following observations:
• It is important that internal procedures are followed and that we take these seriously (such as hot work permits).
• Oil spills or other debris increase the risk of fire.
• Fires can start a long time after hot work is completed.
