An officer on watch was on the bridge watching music videos on his mobile phone and did not notice that the ship was off course. The ship subsequently went aground near to a lighthouse. The case is taken up in the British Safety Digest in its first issue this year. Fatigue was also a contributing cause of the accident, which occurred at night-time.
